Position Highlights

HealthPlus Staffing is seeking an experienced Nurse Practitioner for a full-time opportunity providing comprehensive primary care to older adults in Stockton, California.

This opportunity operates within a PACE (Program of All-Inclusive Care for the Elderly) model, emphasizing coordinated care, chronic disease management, and support for medically complex seniors.

The position offers an 8:00 AM–5:00 PM schedule, approximately six daily patient visits, and no call responsibilities.

Practice Details

• Full-time schedule: 8:00 AM–5:00 PM

• Average patient volume: Approximately 6 visits daily

• No call responsibilities

• Intus EMR

• PACE model of care

• Frail and elderly patient population

• Interdisciplinary, team-based clinical environment

• Preventive care and chronic disease management

• Comprehensive assessments and individualized care planning

• Coordination across outpatient, hospital, nursing facility, and home settings as needed

• Home visits when clinically necessary

Clinical Opportunity

• Perform comprehensive patient assessments and physical examinations

• Diagnose and manage acute and chronic medical conditions

• Develop and implement individualized care plans

• Order and interpret diagnostic testing

• Prescribe medications within authorized scope of practice

• Provide preventive healthcare and patient education

• Coordinate specialty referrals

• Monitor and manage medically complex older adults

• Collaborate with physicians and interdisciplinary healthcare professionals

• Participate in discharge planning and care coordination

• Maintain accurate clinical documentation

• Participate in quality-improvement activities

Compensation & Benefits

• Annual Salary: $140,000–$160,000

• 3 weeks of PTO

• 1 week of CME time

Qualifications

• Graduate of an accredited Nurse Practitioner program

• Current California RN and Nurse Practitioner licenses

• DEA registration required within six months of hire

• Current CPR/BLS and First Aid certifications

• Five years of nursing experience

• At least one year of experience working with a frail or elderly population

• Supervisory experience preferred

• Experience in interdisciplinary healthcare preferred

• Strong physical assessment and chronic disease management skills

• Knowledge of applicable healthcare regulations

• PACE or community-based healthcare experience preferred

• Strong communication and collaboration skills

• Visa sponsorship may be considered but is unlikely and not guaranteed
